Individual Funding Requests
The Individual Funding Request (IFR) process is in place to consider the NHS funding of treatments/interventions/therapies and surgery that is not routinely commissioned by Herefordshire & Worcestershire CCG. The clinician intending to provide this treatment/intervention should only make an application to the CCG after they have reviewed the current policy list.
An IFR application will be considered on the basis of the evidence supplied to confirm that the patient's clinical prestentation and circumstances are exceptional. If this is established, the Panel will review the evidence supplied to confirm the clinical efficacy of the requested treatment for the patient. If this second stage is confirmed, the Panel will then review the evidence of the financial efficacy of the requested treatment.

Please note that some services or interventions are commissioned directly by NHS England. They have their own IFR process, which you can review via their website: www.england.nhs.uk.
Prior Approvals - Some treatments and interventions are commissioned based on the clinical assessment of the patient's condition and symptoms against agreed clinical eligibility. Where this is the case, the CCG has developed a prior approval process that works in line with the published commissioining policies.
